Observation

State what the source actually shows. Keep dates, authorship, and the distinction between direct knowledge and reported information.

Interpretation

Label the explanation. Include reasonable alternatives and the evidence that could distinguish them.

Authority

Read the actual law, agreement, permission, or order. Concern, proximity, support, silence, and role labels do not by themselves grant unrelated authority.

Response

Keep a factual correction possible within the purpose, audience, permission, and exceptions of the channel. Do not force a public disclosure to make a private correction.

Contrary evidence

Keep the rejected claim, later decision, limitation, and unresolved question beside the supporting material.

Return

Name what happened next. Receipt, review, decision, implementation, and verified repair are different events.

A useful note can be very small.
